Perhaps because the appearance of a new person is necessarily the fruit of love. Not only the love of a man and a woman. Practice shows: if a region, the leaders of the region, and not just the family itself, are interested in the birth of a new person, then the demographic statistics become positive. Having many children brightens life. We are talking about this with the chief gynecologist of the Russian Ministry of Health, academician Leila Adamyan.
Leila Vladimirovna, you have been in obstetrics for more than half a century. Your profession, and even more so your position and experience, is the search for creating favorable conditions for conception, for the birth of a child. Of course, it’s easier to say: God gave. But we, and not only atheists, know that this is not so?
Leila Adamyan: My whole life and work are connected with the birth of a person. And I can say that very few people consciously refuse childbearing. They refuse exactly until they meet their betrothed.

But I know firsthand that even couples who love each other, even those who dream of having a child, often postpone the birth of a child, waste time for the simple reason that there is not enough love from the outside. Next week the next 19th International Congress on Reproductive Medicine opens. With the same location – in Moscow. And with the fact that you are its main curator. Recently, it is especially striking that neither sanctions nor iron curtains prevent the number of applicants for new ideas and the exchange of experience. I know that official invitations are not even sent. It’s just that people involved in the “fruits of love”, of their own free will, despite any obstacles, try not to be late for the opening. And they actively participate in its meetings. Each such congress, of course, provides a report on what has been done. However, the main thing is information about new methods, new directions of work.
Leila Adamyan: Otherwise, why such meetings? Leading world experts from 18 countries are participating in our congress.
I was lucky: more than once at such medical forums I learned about completely new approaches to so-called women’s problems. Including, for example, the use of the Da Vinci robot. Now this is not the only robot used in medicine. Robots from South Korea were very welcome. But you, gynecologists, are still faithful to Da Vinci as the first and proven one?
Leila Adamyan: By and large, robots have become commonplace. They are, or at least should be, in all major medical institutions. Congress these days is a mandatory presence in operating rooms, an opportunity to follow every move of doctors and nurses, and to observe the patient’s condition literally with their own eyes. This is a given.
I agree. But when space surgery is announced, as they say, give us the details! What is it?
Leila Adamyan: Space medicine, in particular reproductive medicine, is a completely new direction that is currently developing. And one of the congress participants, international expert Atila Velecki, will talk about reproductive medicine in space. More specifically? For example, IVF in space.
IVF in space? Did you make a mistake? What do you mean? Are women offered IVF in space? What, is it more effective in space? Should applicants for this procedure also go into space? Absurd…
Leila Adamyan: Not absurd! In the future, if not even the short term, this may be relevant taking into account long-term space expeditions, lunar bases, etc. Because a man and a woman, including married couples, can stay in space for a long time. And their personal life must continue. And the desire to have a child does not go beyond the boundaries of the Galaxy. We need to make sure that IVF becomes possible in space. This opinion, although it may seem controversial and ambiguous to some, is not limited to groups of scientists. This is the need of the hour. Why not?
Let’s leave the congress. But let’s continue about IVF. In vitro fertilization, the possibility of giving birth to a baby using this procedure, is becoming increasingly popular. Or am I mistaken? Is the situation with IVF different now than it was before? Is it a problem to even obtain permission to carry out this procedure – or rather, a quota? Is this a lot of money? Or are there no obstacles?
Leila Adamyan: Today in Russia a huge percentage of IVF is carried out within the framework of compulsory medical insurance. This is a huge achievement for our country. IVF is a great blessing for those families in which pregnancy does not occur naturally.
Let us remind you that quotas are applied for by those who are desperate to get pregnant naturally, those whose age is off scale. By the way, Leila Vladimirovna, at what age is it best to give birth? I ask because even around me there are three ladies who, to put it mildly, are over 40 and already have children of school and university age, are preparing for IVF.
Leila Adamyan: All ages are submissive to love. As for IVF… Yes – this is a very modern and proven technology. With its help, all over the world and, of course, in our country, the number of successful pregnancies is growing. I’ll give you some statistics. Let me just emphasize first: those who most often apply for IVF are those who have suffered serious illnesses or have undergone difficult surgical interventions. Those who have a congenital or concomitant pathology that prevents pregnancy. Endocrine pathology, immune diseases. Although age, of course, is significant. In any case, it reduces the chances of achieving pregnancy. This applies to both women and men.
Let me remind you of a common truth: if you want to become parents, you must contact us in time. Alas, the egg ages rapidly. The “golden age” of the egg is up to 37 years. After this, their quantity and quality drop dramatically and forms are formed, in some cases degenerative, unsuitable even for IVF. It is no coincidence that a new direction has appeared: over the past years we have been carrying out the prevention of premature aging of the ovaries.

Leila Adamyan: We are on the right track. It is gratifying that this direction is developing not only in federal, but also in regional centers, in Moscow. There is a special project “determination of ovarian tissue reserve”. This means that women of reproductive age should have blood tests for hormones, anti-Mullerian hormone levels, and ultrasounds to determine the number and size of follicles. This makes it possible to predict future reproduction.
The fact is that there are often cases when very young women complain of infertility and other very important symptoms of premature aging. All over the world, special technologies for examining such women and measures of assistance, including surgical interventions, are being developed.

I noticed that your report, Leila Vladimirovna, is devoted to the development of methods to combat such “intimate” aging – on the role of surgical activation in this pathology. Can artificial intelligence help you here?
Leila Adamyan: Certainly! Let’s say, with the help of artificial intelligence, we have the opportunity to monitor the behavior and development of an embryo, placing it in a special chamber until it is transferred to the uterus. Moreover, the embryo can be observed from thousands of kilometers away from the embryo itself. And not only observe, but also create the right conditions for its development.
And it has nothing to do with the weather outside. What about an increase or decrease in the birth rate? A young nurse I recently knew, who some time ago shared that she was preparing to conceive, now suddenly said: she’ll wait until… normal weather settles. I read somewhere that children born in spring or summer are more prosperous than those born at other times of the year.
Leila Adamyan: Nature doesn’t have bad weather. And nowhere in the world are there statistics cited by a young nurse. But there are other statistics. Reproduction is influenced by the age at which the first child is born. After all, a young, healthy, normally functioning body – this applies to both men and women – has the ability to both conceive and bear a healthy child. Yes, and become a parent several times. This is the standard for a successful solution to reproductive problems.
From 20 to 40 years, both he and she realize their reproductive potential. At the same time, they do not put themselves and the unborn child at risk. No wonder they say – in a healthy womb there is a healthy child. The criterion for reproductive health is the age at which the first child is born.

Leila Adamyan: In European countries, the age of birth of the first child is approaching 35-40 years. This is the way to reduce the birth rate in general. On the contrary, in eastern countries the statistics are different: the first child is born, as a rule, to young parents – at 20-22 years old. And by the age of 40, many couples begin to have many children.
It is known that after 40 years, many concomitant diseases appear, not only gynecological ones, which, naturally, have a detrimental effect on the development of the entire organism. Especially for reproductive health. It is appropriate to remind you: it is not at all indifferent whether dad, and especially mom, smokes or not. Dad or mom drinks. Not to mention drug addiction and other bad habits. All this, like poison, affects the unborn child.
Maybe the question is not entirely on topic. But I will ask. Endless information about how a man, to put it mildly, at an “overripe” age, finds a young girlfriend and is proud of the birth of an heir. Some, however, do not believe such messages. But such cases, you see, do happen. Are all ages submissive to love? What is your attitude?
Leila Adamyan: Yes, all ages are submissive to love. But, unfortunately, with age, problems accumulate in the body, not only in women, but also in men. Of course, not all people are in the same physical shape. It’s not for nothing that there is biological age and actual age, that is, according to the passport. And it’s not just what a person looks like. But also the state of health. And in 50 percent of cases it depends on lifestyle, ecology, actions, wear and tear. 20 percent – from the level of medicine, 15 percent – from genetics, which can sometimes be deceived, knowing its subtleties. And 15 percent are associated with natural disasters and epidemics. This has been proven.
And finally. The fruit of love… Each region, mayors and, of course, doctors, and each of us, are obliged to take care of increasing the birth rate, fulfilling the most important task: Russia must become a country with many children. By the way, it is very good for health. And it will never become outdated, even in our digital times: a large family is reliable insurance for the rest of your life.
