Integrated evaluation of high blood pressure and diabetes clinics… 32.2% and 36.3% of excellent institutions, respectively

(Health Korea News / Lee Chang-yong) As a result of conducting an integrated evaluation of hypertension and diabetes among clinic-level medical institutions, it was found that 32.2% and 36.3% of all clinics received excellent grades (grades 1 to 2), respectively.

On the 27th, the Health Insurance Review and Assessment Service released the first results of the integrated evaluation of hypertension and diabetes clinics through the HIRA website and mobile app.

HIRA has previously evaluated the adequacy of high blood pressure and diabetes separately for each disease, but from 2023, it has improved (second cycle) to an integrated evaluation that includes patients with high blood pressure, diabetes, and complex diseases, and released its first evaluation results this year. HIRA plans to provide incentives to excellent institutions (1st to 2nd grade institutions).

The first hypertension and diabetes adequacy evaluation of the second cycle, which is being released this time, was conducted from March 2023 to February 2024 on 24,640 clinics that prescribed out-of-hospital blood pressure or blood sugar-lowering medications due to high blood pressure or diabetes.

On this day, the HIRA disclosed the disease-specific and institution-specific grades of all institutions (clinics) subject to evaluation, categorized into grades 1 to 5, in order to provide the public with the information they need when choosing a clinic near their home.

The grade for each disease is the evaluation grade for each disease, such as high blood pressure or diabetes. The institutions that achieved grade 1 or 2 for each disease were 7,795 hypertension treatment institutions (32.2% of all clinics) and 6,609 diabetes treatment institutions (36.3% of all clinics) nationwide.

The institutional grade is a grade that reflects the patient volume in the grade for each disease. A total of 7,296 institutions (29.6% of all clinics) nationwide were found to have achieved institutional level 1 or 2.

The improved evaluation criteria in this integrated evaluation are a total of 11 indicators: ▲common indicators for comprehensive management of patients with hypertension, diabetes, and complex diseases (2) ▲test indicators reflecting the characteristics of each hypertension and diabetes disease (7) ▲outcome indicators (2) c.

Common indicators (2) were calculated for the first time after evaluation integration. As a result of the evaluation, the proportion of patients who continued to visit for all subjects with hypertension and diabetes was 87.6%, and the proportion of patients who continued to take prescriptions was 84.1%.

The inspection indicators (7) are evaluation indicators (implementation rates) for the prevention and management of complications of each disease. The implementation rate of each indicator in hypertension was 73.3% for blood test, 47.0% for general urine test, and 35.9% for electrocardiogram test. In the case of diabetes, ▲glycated hemoglobin test 69.4% ▲lipid test 82.3% ▲diabetic nephropathy screening test 28.0% ▲fundus test 43.6%.

An official from HIRA said, “Inspection indicators have been continuously increasing since the introduction of the evaluation,” and “the results of this evaluation showed an increase of 3.4 to 6.6 percentage points compared to the number of vehicles in all indicators.”

The result indicators (2) are new indicators for the 2nd period. The evaluation was conducted on 1,312 clinics that selected the relevant indicators. The blood pressure control rate for hypertension was 66.6%, and the glycated hemoglobin control rate for diabetes was 64.1%.

HIRA plans to provide incentives to excellent institutions according to the results of this integrated evaluation of hypertension and diabetes. The target is clinics (8,403 locations) with high blood pressure or diabetes level 1 or 2, and the total amount is 27 billion won. Additional incentives will also be paid to lawmakers (1,060 locations) who separately selected outcome indicators.

Kang Jung-gu, Director of the Review and Assessment Service, said, “Hypertension and diabetes are representative chronic diseases, and consistent management is important to prevent complications such as cardiovascular and cerebrovascular diseases. Accordingly, we hope that the results of this evaluation will be actively used by the public in choosing a clinic near them,” he said, adding, “We will strive to improve the autonomous quality of medical services for hypertension and diabetes management through the advancement of evaluation indicators.”
