The College of Physicians warns the authorities. The new management of CMR adopts a much firmer position in the negotiations with the National Insurance House.

Univ. Prof. Dr. Cătălina Poiană, the new president of CMR, draws attention that any decision with an impact on the medical profession must be discussed with the representatives of doctors.

“We consider it necessary to have a real dialogue and a transparent negotiation between the state institutions and the College of Doctors. It is not normal for an important document to be published without us being consulted”, says Dr. Cătălina Poiană

CMR also highlights the problems raised by the lack of indexation of funding with the inflation rate and the new fiscal challenges for medical practices. CMR representatives sent official requests for meetings with CNAS, including the proposal of a consultation calendar, but did not receive a response, according to DC News.

“Respect for Doctors starts from the respect of state institutions for doctors!

The new management of the College of Doctors in Romania draws attention to the fact that a decision-making transparency document was posted on the website of the National Health Insurance Company without any dialogue or negotiation with the doctors’ representatives beforehand.

The document refers to the extension of contracts for the first quarter of 2025 in all fields of medical assistance.

On the website www.cnas.ro (the official platform of the National Health Insurance Company – CNAS) has been posted, since the end of last week, in decisional transparency, a document that was not negotiated with the representatives of the College of Physicians in Romania (CMR) regarding the extension of contracts for the first quarter of 2025 on all fields of medical assistance. The document did not take into account the interventions of the CMR representatives from the working groups on the amendments to the CoCa and Norms, nor on the financing.

The Romanian College of Physicians expresses its concern regarding the maintenance for the first quarter of 2025 of the value of the points for medicine in the specialized outpatient clinic and primary medicine, in contradiction with the current economic and social situation, with the financial forecasts and, above all, with the discussions previously held with CNAS on financing.

Regarding the CNAS proposal to eliminate regularization at 11 months, CMR representatives request the reintroduction of the regularization mechanism, as it was in the 2023 contract, only in this way the amounts allocated to specialized ambulatory care and primary medical care will be able to be fully used in these sectors of medical assistance.

At this moment, due to the implementation of the electronic invoicing system, it is no longer possible to observe and quantify the total number of consultations performed, doctors thus performing an important number of pro-bono medical services.

The representatives of the College of Doctors from Romania also specify:

  • The CMR agreed to maintain the value of the points for the specialized outpatient clinic and primary medical assistance only for the year 2024, with the aim of ensuring the stability and continuity of the medical assistance provision;
  • The CMR agreed with the elimination of the regularization at 11 months for the specialized ambulatory and primary medical assistance only for the year 2024, understanding the financial difficulties of this year and the fact that the state budget had already been adopted;
  • The tax code was changed for 2024 and will undergo substantial changes for 2025 as well, increasing the tax burden for medical practices. On the other hand, the increased taxation created the framework of a more comprehensive FNUASS, therefore of an increased allocation;
  • We believe that the state budget for 2025 must be adapted to the real needs of the health system with clear allocations necessary for the more efficient functioning of the health system;
  • From July 1, 2023 until now, the financing of primary medical care and specialized outpatient clinics has not been indexed to the inflation rate, even if the expenses necessary for the operation of medical offices have permanently increased, regardless of their form of organization. This fact did not allow the development of practices, but only their functioning at the limit.

“In order to correctly represent the interests of doctors, as stated in the CMR regulations, we believe that there is also a need for an openness and a true interest in dialogue and negotiation between state institutions and the CMR.
